The ride takes about 30 minutes in light traffic, though in rush hour it can push closer to 50 minutes as you hit the A8 into the city.
Buses leave roughly every 30 minutes from both T1 and T2, starting early morning and running into late evening (timetables shift slightly by season, so check the day before). At T1, look for the bus bays outside the arrivals area on the ground floor; at T2, the stop sits just outside arrivals near the exit doors by the taxi rank.

The bus drops you at Milano Centrale on the Piazza Luigi di Savoia side, which lines up well for Trenitalia and Italo departures to cities like Bologna, Florence, and Rome. From that same side of Centrale, you also have access to Milan Metro lines M2 (green) and M3 (yellow), which gets you to Cadorna, Duomo, or Porta Romana in 10–20 minutes depending on the transfer.
The trade-off versus a taxi (usually €90–€100 to central Milan, about 45 minutes) is unpredictability: if you land into MXP around 08:00 or 18:00, expect heavier traffic and a slower run than the advertised 30 minutes.
